Position Description

Seeking an experienced Nurse Practitioner Supervisor to oversee and coordinate the care of participants in both clinic and home care settings in Manhattan and the Bronx.

Setting: In-person clinic, some field (home care)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (1 hour lunch)
Contract Length: Ongoing

Responsibilities:

  • Supervise and manage Nurse Practitioners at assigned locations.
  • Perform wellness assessments and manage acute care needs for participants.
  • Ensure timely follow-up for patients post-hospitalization.
  • Provide urgent care services and home visits as needed.
  • Collaborate with physicians and the interdisciplinary team to develop and implement care plans.
  • Conduct clinical documentation and ensure compliance with regulations.
  • Participate in team meetings and offer feedback on patient care.
  • Manage complex care for high-risk participants.
  • Provide emergency coverage and participate in on-call rotation.

Qualifications:

  • Education: Graduated from an accredited Nurse Practitioner program.
  • Experience: Minimum of 2 years in clinical practice, with at least 1 year working with frail or elderly populations.
  • Licensure: Active NY Nurse Practitioner license, DEA Certification, and AANP/ANCC Board Certification required.
  • Skills: Strong communication, analytical, and organizational skills.
  • Additional: Must be legally authorized to practice in New York and have Medicare/Medicaid Provider IDs in good standing. Bilingual preferred.
