Lead Family Nurse Practitioner - Outpatient Primary Care

Minimum 5 years of experience required

An established and community-centered outpatient practice in Claremont, New Hampshire, is seeking a Family Nurse Practitioner to lead their growing primary care team. Practice Details:

• Modern Outpatient Facility: 13 exam rooms, including dedicated pediatric and procedure spaces
• Comprehensive On-site Services: Diagnostics, imaging, minor procedures, and wellness programs-all under one roof
• Team-Based Care: Work alongside 3 other FNPs, 1 PA-C, experienced MAs/LNAs, and a radiology technician available 5 days a week
• Tech-Enabled Efficiency: Utilizes Athena EMR with integration into Dartmouth's EPIC system
• Schedule: Monday-Thursday with the clinic closed on Fridays; phone-only call rotation one day per week
• Growing Patient Panel: Currently managing care for approximately 5,000 patients and actively accepting new ones

Compensation & Benefits
