One of our client is looking for a Nurse Practitioner

Type: Direct-Hire

Location: Oceanport, NJ

Job Summary

The PCP Nurse Practitioner has the responsibility for providing direct and indirect care to the participants in the PACE Health Clinic and Day Center.

Works under the direct supervision of the Medical Director/PCP.

Manages complex clinical situations for which he/she is responsible.

Essential Duties And Responsibilities

Performs health histories, physical exams, and participant teaching.

Orders, interprets, and follows up on lab tests and results.

Performs selected procedures and documents care in writing in EMR.

Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others.

Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within attending physician approved protocols.

Assesses, initiates, and coordinates participant care on a daily basis under the supervision of the attending physician.

Makes referral to other physicians including specialists with staff PCP approval.

Requests consultations from other services as needed in collaboration with attending physician and/or established protocols.

Acts as liaison with physicians, nurses and other healthcare professionals providing education, communication and support for healthcare staff, participants, and families.

Directs participant management in conjunction with physician staff.

Participates in the PACE provider call schedule.

Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.

Assists the IDT in formulating a plan for the coordination of 24-hour care.

Documents participant changes appropriately in the medical record and communicates participant changes to team members.

Evaluates and treats participants in their homes, assisted living facilities, nursing homes, and hospitals when medically indicated.

Precepts medical students and residents during their PACE rotation.

Encourages all participants to have Advanced Directives and provides education as needed; may discuss the Do Not Resuscitate (DNR) option with the participant and/or family members and may complete the Health Care Wishes form and write DNR orders in the medical record, in collaboration with a physician

Requirements

Master's degree in clinical nursing from accredited Nurse practitioner program. HR Job Description

3-5 Years Nursing Experience With Nurse Practitioner Experience Preferred.

RN licensure in perspective state and a Family certification from Nurse Practitioner program

  • Only act within the scope of his or her authority to practice.

Meet a standardized set of competencies for the specific position description established by the state and approved by CMS before working

1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring

  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.

Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.

Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.

Skills And Abilities

Ability to learn, team and research; ability to solve problems and think analytically; must possess skills necessary to treat geriatric participants; ability to get along with others in a team environment. independently.

High level of autonomy is required for clinical and management decisions while reporting to PCP and Center Director.

Ability to work a flexible work schedule.

Knowledge of physical, mental, and social needs of frail older adults.

Effective skills in physical assessment and chronic disease management for frail older adults.

Able to work within the Interdisciplinary Team (IDT) setting.

Able to utilize basic computer skills in the workplace.
