Nurse Practitioners (NPs & FNPs) – Telehealth | Flexible Schedule

Path Care Health is not your typical telehealth company — we’re the concierge of telehealth. We partner with home care agencies to provide fast, reliable, and high-quality virtual assessments that ensure patients are ready for home care enrollment with zero delays. Our admin team handles all scheduling and follow-up, so our providers can focus on what they do best: patient care.

Position Overview:
We are currently seeking licensed Nurse Practitioners (NPs and FNPs) to join our Team

What You’ll Do:

• Perform virtual assessments for patients needing home care services

• Complete all forms and documentation with same-day turnaround

• Collaborate with care coordinators, intake teams, and agency staff

• Maintain accurate and timely clinical notes

What We’re Looking For:

• Active NP or FNP license (NY preferred; other states also encouraged to apply)

• Background in home care, primary care, or geriatrics is a plus

• Excellent communication and time management skills

• Comfortable with basic tech and virtual care platforms
