Our client, a Federally Qualified Health Center (FQHC) with multiple locations in New York City, is hiring a Family Nurse Practitioner. Play a vital role in providing comprehensive primary care services to patients, focusing on wellness visits, disease management, and prevention. Work collaboratively with a multi-disciplinary team to deliver high-quality medical care, addressing the complex needs of patients with comorbidities such as diabetes, hypertension, and HIV. Your expertise will be essential in delivering specialized medical care, including gender-affirming care and opioid addiction treatment.
Responsibilities

Play a vital role in providing comprehensive primary care services to patients, focusing on wellness visits, disease management, and prevention. Work collaboratively with a multi-disciplinary team to deliver high-quality medical care, addressing the complex needs of patients with comorbidities such as diabetes, hypertension, and HIV. Your expertise will be essential in delivering specialized medical care, including gender-affirming care and opioid addiction treatment.
Responsibilities
- Provide primary care services as part of the primary care clinical team
- Conduct wellness visits, manage care, and prescribe/administer treatment
- Analyze records, reports, and test results to prevent and/or diagnose illness, disease, or injury
- Collaborate with other healthcare professionals to deliver medical services
- Manage labs, referrals, and other job-related duties as required
- Current certification as a Family Nurse Practitioner (FNP)
- Master's degree in Nursing (MSN) with a focus on Family Nurse Practitioner
- 1-2 years of experience in primary care, preferably in a clinical setting
- Strong clinical skills, including assessment, diagnosis, and treatment
- Excellent communication and collaboration skills
- Ability to work in a fast-paced environment with multiple priorities
- Familiarity with ECU EMR system (training provided if necessary)
- Experience with population health management and care coordination, preferred
- Knowledge of LGBTQ+ health and wellness, preferred
- Bilingual or multilingual skills, preferred
